Executive Summary
- Sustainability and transformation partnerships (STPs) were first announced in December 2015 as part of the NHS planning guidance, as a new approach to help ensure that health and care services are built around the needs of local populations
- To create the plans, organisations across the country are coming together to design “place-based plans” which will ensure the sustainability of health and care services in their area
- Matthew Swindells’ Directorate for operations and information has been expanded to include overall delivery of the Five Year Forward View, including STPs and new care models. The change marks a shift from strategy development to practical implementation of the Five Year Forward View
